Case Management Program

Guidelines

The Case Management Program is an extension of the Blanchet House of Hospitality Residential Living and Transitional Housing Program. Upon entrance into the Program a guest will meet with a case manager in the following capacity:

Intake:

  1. Case Manager will meet with guest the day he enters the Blanchet House of Hospitality Residential Program to complete case management intake information, case management attendance policy, and to sign release of information form(s).
  2. Guest will be given a list of coping skills, reflection journal exercise, calendar for time management, and resource information. CM and guest will agree on his follow through of 1-3 goals or tasks for the duration of the weeks.

First 3 Months:

  1. Case Manager will meet with guest one time per week.
  2. Case Manager will present Life Plan to guest (life plan will have 8 components representing recovery and guest will be able to identify his goals and or tasks to focus on in each of these areas)
  3. Life Plan components include the following:
    • Medical
    • Emotional
    • Recovery
    • Financial/Legal
    • Social
    • Spiritual
    • Occupation/Education/Housing
    • Maintenance

Second 4 Months:

  1. Case Manager will meet with guest one time every two weeks.
  2. Case Manager will continue to guide and support guest in development and completion of Life Plan.

Transitional Living:

  1. Case Manager will meet with guest one time every two weeks.
  2. Case Manager will continue to guide and support guest in follow through of the maintenance stage of his Life Plan and commitments he made in his petition to move to the 4th floor transitional housing program.
